Problems adding new customer

I am working to get the Freeside up and running.

When I tried to add a new customer, I get the following error:

Internal Server Error

The server encountered an internal error or misconfiguration and was
unable to complete your
request.

Please contact the server administrator, webmaster@freeside.hacom.net and
inform them of the time the error occurred, and anything you might have
done that may have caused the error.

When I looked the log file, I found these
