[freeside] Customer Add Error: Illegal of empty (numeric) domsvc

Ok, after stumbling across a few problems, i opted to restart the
freeside install 
for 2nd time, following everything in to par in the docs/admin.html
file, until you
get to the section where you just completed adding your new customer.

You get taken to a Customer View Screen and order this 'domain
definition' package 
you were asked to create, still all is fine until the next point.

Which is : From the Package View screen of the newly created package,
choose (Add) to add the customer's service for this new package. 
Add your own domain. 

Below I have included a copy of the Package View screen, and i for the
life of me have no
idea where tihs 'ADD' or how to get to an 'ADD' to do what it says.

It looks like you've added a service definition for a domain, but not
actually provisioned one.  Also, you probably don't want to set domain
to be a fixed value in the service definition - I doubt all your
provisioned domain services should have the same domain. 

Follow all steps in httemplate/docs/admin.html.
