[freeside-users] FS-4.0 CENTOS 6.7

Thanks Obinna, I had already done these steps

1. make install-apache to replace the apache snippet with exactly like
attachement
2. deleted the .htaccess in the docs folder to make sure basic
authentication is not used
3. created a non-htpasswd admin with password

I still get bad cookie on upgraded install. And new install has empty
access tables with error:

DBD::mysql::st execute failed: Cannot add or update a child row: a foreign
key constraint fails (`freeside`.`access_usergroup`, CONSTRAINT
`access_usergroup_ibfk_2` FOREIGN KEY (`groupnum`) REFERENCES
`access_group` (`groupnum`)) [for Statement "INSERT INTO access_usergroup (
usernum, groupnum) VALUES ('6', '1')"] at
/usr/local/share/perl5/FS/Record.pm line 1375.
Cannot add or update a child row: a foreign key constraint fails
(`freeside`.`access_usergroup`, CONSTRAINT `access_usergroup_ibfk_2`
FOREIGN KEY (`groupnum`) REFERENCES `access_group` (`groupnum`)) at
/usr/local/bin/freeside-adduser line 39.
