fatal: Can't get REMOTE_USER

Both install.html and config.html from the documentation link to the
Apache FAQ "How do I set up Apache to require a username and password to
access certain documents?" at
<http://www.apache.org/docs/misc/FAQ.html#user-authentication>, where
there are links to both the concise apache.org documentation and excellent
tutorial/examples at Apache Week. 

Okay, It seems the current documentation is confusing lots of people.  Any
changes/clarifications are welcome.
